As you would expect from any friend or family member, all on your Home & Away Care team are compassionate, kind and enthusiastic. All on the team are drawn from the local and broader community and all are skilled and knowledgeable. They will support you in a way which meets your needs and aspirations. We recognise that it is essential to ensure we have the right people with the right attitudes, skills, values and experience, providing the high standard of care you deserve. We work to ensure that all on the Home & Away care team are adequately up skilled to meet your specific needs which is why ‘we recruit the best and train for excellence’ .
When you invite us to join in your care we will discuss with you your needs and expectations. You may want companionship and a helping hand with some or all of the activities of daily living such as moving about, dressing, eating and light housekeeping. You may also wish to try and do some of these tasks yourself but are not able at present. You may have recently been admitted to hospital or suffered a period of illness or injury and are concerned about managing at home. You may have difficulty with:
- Preparing meals
- Cleaning and shopping
- Getting out of and going to bed
- Taking your medication
- Washing and going to the toilet
We will agree with you how we can support you to do these things independently and work with you (ReAblement) to reach your goals and independence at home and in the community.
You may require specialist Nursing Care at home while recovering from an illness or injury or following hospitalisation. The Home & Aware Care team of carers, nurses, doctors and therapists– your Care Family, will assist in your recovery and help manage your health problems –some of which can only be safely carried out by a Nurse such as managing difficult leg ulcers which need special dressings or care of a catheter to helping you manage your diabetes, cardiac condition, arthritis, dementia, bronchitis, physical disability etc. We will assist YOU to understand and participate in creating a health care plan that works for you .
